Description
Bias in mental health practice is rarely a matter of anyone deciding to discriminate. It shows up in which symptoms get
explored and which get assumed, whose account is believed, which intake form has a box for your family, and which crisis
gets a clinician rather than a police officer. This course moves past awareness and into examination. After clarifying the
terms the field uses loosely and reviewing what the research shows about how bias operates and what it costs,
participants work through twenty brief case studies drawn from patterns documented in the clinical and health-services
literature. Cases are organized by ecological level — micro, mezzo, and macro — and span race, ethnicity, religion, gender
identity, sexual orientation, age, weight, parenthood, disability, immigration status, and socioeconomic status.

- 1.Distinguish implicit bias, explicit bias, and structural, institutional, interpersonal, and internalized racism, and describe how each operates in mental health practice.
- 2.Identify how bias surfaces in the clinical encounter through microaggressions, diagnostic overshadowing, testimonial injustice, and stereotype threat.
- 3.Describe the documented impacts of bias on clients and professionals, including minority stress, racial trauma, allostatic load, weathering, and therapeutic mistrust.
- 4.Analyze case examples at the micro, mezzo, and macro levels to locate where bias or structural racism influenced assessment, diagnosis, treatment planning, or engagement.
- 5.Apply cultural humility, critical consciousness, anti-oppressive practice, intersectionality, and positionality to name at least one concrete, equitable alternative in each case.

therapeutic and medical mistrust. How discrimination becomes
embodied, and why mistrust is a reasonable response.

Bias
Cultural humility, critical consciousness, anti-oppressive practice,
intersectionality, and positionality — with participants naming what
they have already tried.

Micro-Level Case Studies The individual clinical encounter: diagnostic overweighting, language
access, religion read as pathology, diagnostic overshadowing, ageism,
weight bias, paternal postpartum depression, microaggression and
alliance rupture, bias directed at the clinician, and client
self-censorship.

Mezzo-Level Case Studies The agency, program, and community level: intake forms that exclude
chosen family, assessment tools normed on narrow populations,
inaccessible facilities, identification requirements at intake, and
insurance-driven session limits.

Macro-Level Case Studies The policy and societal level: crisis response and involuntary holds,
licensure barriers for internationally trained clinicians, culturally
responsive care for Jewish clients, state restrictions on
gender-affirming care, and reimbursement structures.
